Fallopian Tubes Contribution to Fertility

The fallopian tubes are a part of reproductive system in the female body. Their function is to collect the egg and by propulsive movements transport the egg towards the uterus. For pregnancy to occur the tubes should be open and they should function normally.

Tubal Factor is implicated in 15-25% of the cases of infertility. Tubes can be damaged i.e. blocked or their normal function can be disturbed leading to improper functioning. Repeated pelvic infections in females, history of tuberculosis, endometriosis, any pelvis mass like fibroid, large ovarian cysts, and history of pelvic radiotherapy for cancers can cause tubal factor infertility. Often, the damaged tubes have fluid collection in them which is known as hydrosalpinx. This collected fluid has harmful chemicals which are toxic to embryo development.

There are various tests to evaluate the tubal patency

  • The most simple is Sonosalpingogram (SSG). It is done routinely in clinic under ultrasound guidance. A very small diameter rubber catheter is placed inside the uterus and saline is injected which comes out of the tubes freely if the tubes are open. There is no cut or stitch in this process. However, the functioning of the tube cannot be seen in tubal patency tests.
  • Another test which helps in visualizing the tubes is known as Hysterosalpingogram (HSG). In this test radiopaque dye is injected inside the uterus and when the dye flows inside the tubes, its photographs are taken with the help of X ray machine. This test delineates the tube clearly and pathologies like tubal distortion, blockage of tube, beading of tubes, convolutions of the tube, presence of hydrosalpinx can be easily made out.
  • Tubes can also be tested during Laparoscopy where a telescope inserted through a keyhole below the umbilicus inside the abdominal cavity and fluid can be seen coming out through the tubes directly. The laparoscopy is an invasive test which involves anesthesia administration, but it has an added advantage that any pathology can be treated in the same setting.

The treatment options for tubal pathology depend on many factors. The age of the female partner, duration of infertility, history of failed treatments, the extent of damage to the tube, presence of hydrosalpinx, patient preferences should be taken into consideration and thorough counseling should be done. Sometimes when the patient is anxious during the tubal test evaluation, the reports can show false tubal blockage , which should be ruled out laproscopically. The HSG film should be read by an experienced infertility expert who can pick up subtle abnormalities and this can make huge differences in the management of the problem.

IVF is offered for those patients where age of the female partner is advanced, couple has a long duration of infertility, multiple IUI failures, ovarian reserve is less, associated male factor infertility is present, the tubes are moderately to severely damaged or there is presence of hydrosalpinx. In IVF, mature eggs are directly picked up from the ovaries with the help of needle placed through the vagina and then each egg is injected with a healthy sperm (ICSI) in the laboratory. The embryos are transferred into the womb. In this way the damaged tubes are bypassed and a pregnancy can be achieved.

In the era of IVF where the tubes can be bypassed, the role of tubal surgery has become limited. However if a couple is young, there is no other infertility factor, the tubal pathology is mild and there is no hydrosalpinx, tubal surgery can be attempted. It should be kept in mind that the results of tubal surgery in the terms of pregnancy, depend on patient profile, extent of damage to tubes, and surgeon expertise. If a hydrosalpinx is present then the tube should be detached from the uterus (tubal clipping) to prevent backflow of toxic fluid from the damaged tubes to the uterus. This procedure is called laproscopic tubal clipping and it is done to improve the chances of pregnancy outcome.

Treatment options for high FSH

FSH stands for follicle stimulating hormone. It is produced by the pituitary gland in the brain. As the name suggests, it stimulates the growth of the follicle inside the ovary. Its level in the blood are in turn regulated by the estradiol and inhibin produced by the growing follicle inside the ovary.

FSH levels are measured along with antimullerian levels( AMH) and antral follicle count( AFC) to assess the ovarian reserve of the patient. As the FSH levels vary throughout the menstrual cycle, it should be done on day2/3 of the cycle and preferably along with estradiol levels for correct interpretation.

As the age advances, the number of follicles decline in the ovary resulting in low AMH and High FSH levels. However in around 10% of the patients accelerated ovarian ageing occur and they present with low AMH and high FSH levels at a younger age. Certain autoimmune disorders , prior radiation , chemotherapy , as well as certain viral infections may cause ovaries to fail prematurely which is defined by FSH levels > 40 mIU/ml .

High FSH and low AMH entails need for aggressive fertility treatment. IVF remains cornerstone therapy in these patients.

The success rate of IVF using self eggs varies from 6% to 30% depending upon the age of the patient and cause of the decreased ovarian reserve. It is in research that patients at risk of developing decreased ovarian reserve may be identified through screening of FMR1 gene premutations. These patients may be offered fertility preservation through ovum or embryo preservation. Patients undergoing chemotherapy or radiation therapy may be offered ovarian cortex cryopreservation.

Before IUI, sonosalpingogram which is a test to know whether the tubes are open or not is done routinely without anaesthesia.

4 to 5 cycles of ovulation induction and IUI can be offered to the patient depending upon the age and other factors.If the number of eggs are less in ovary then treatment should be at earliest and aggressive, therefore IVF is offered. If both the tubes are blocked then, IVF is the only solution.

What is IUI? How is it done?

It is a technique where semen is prepared, concentrated and put inside the womb at the time of egg release . It has the advantage of directly putting sperms which are good in number and highly motile closer to egg . It is an office procedure and is done without any anaesthesia. There is no cut or pain . The patient needs to rest for 15 minutes and can carry out her daily routine .

How many cycles can I try for IUI?

The chances of conceiving are maximum within first 4 cycles of IUI. However a maximum of 6 cycles can be done depending upon age and ovarian reserve of the patient.

What are the chances of us getting a baby?

In IUI chances betwen 10% to 15% per cycle. In IVF - a chance of 40% to 45% per cycle is seen.

What is IVF & ICSI? How is it done?

IVF is the technique where dugs are given to the female partner to produce many eggs . The eggs are then collected through vagina . The procedure is called ovum pick up and it is done under general anaesthesia. There is no cut or no stitch . The eggs are then mixed with sperm to make babies. When the eggs are incubated with semen in a petridish then fertilisation takes on its own , then the procedure is known as IVF( In vitro fertilisation). However , when one sperm is injected inside each egg then it is known as ICSI ( intracytoplasmic sperm injection). The embryos are kept in incubator and transferred into womb at day 3 or day 5 depending upon the number and quality of the embryos.

What is the duration of the treatment?

The stimulation of the female takes 9 to 10 days. It is started from day 1 or day2 of the cycle. Then the ovum pick up occurs 34-36 hours after the trigger which is the final injection to mature the eggs. The embryo transfer occurs on day 2 to day 5 after the pick up. So the whole process takes around 1 2to 15 days. Sometimes, when the female produces a large number of eggs, especially in case of polycystic ovaries, then the embryos are frozen and transferred into another cycle, and it is known as Frozen embryo transfer cycle.

What precautions do we need to take before starting the treatment?

The female is advised to take healthy diet and regular exercise . Good sleep, yoga/meditation helps to relax oneself. Smoking and alcohol should be avoided.

Complete Bed Rest improves the chances of success of IVF?

BURSTING yet another very common myth surrounding IVF treatment is that one needs complete bed rest after the embryo transfer. The IVF is a time-taking and costly procedure. Hence after embryo transfer, many women are afraid that their careless actions can lead to failure. Most women are worried that their chances of conceiving may decrease if they do not take bed rest or resume their day to day activities.

However, the fact is that there is no need to put your life on pause after the procedure. The idea of bed rest is an absolute myth. 'Fertility and Sterility', a study reported in 2013 by Gaikwad, Garrido et al. (Spain), reports that 10 minutes of bed rest could be essential post the procedure. The study included two groups of women, the first, who was told to take rest for 10 minutes post the procedure and the second, who was discharged immediately. The results concluded that the second group women, who were discharged immediately post the transfer had an 18% higher success rate and a 10% decrease was also observed in the spontaneous abortion rate, in case of the first group women who took rest after the procedure.

Let's Understand, the embryo will not fall out of the womb as you move or stand up. We have to understand that the endometrium has its own movements and patterns. When the embryo reaches the uterus, it keeps it inside the uterine cavity, hence preventing it from expulsion or implantation in the inappropriate area. Therefore, it is important to understand that implantation does not depend on women's posture or movement.

Movement, however, plays an important role in blood circulation. The longer a person lays down in bed, the slower the blood circulation is, which further affects the blood circulation to the uterus, which can hamper the growth of the embryo.
